Monitoring Plan Selection

Customer selects one of the following monitoring terms at a rate of $36.00 per month:

☐ 36 Month Monitoring Agreement (Best Value)

1. Agreement Term

This Agreement begins upon activation of monitoring services and will remain in effect for the full duration of the selected term above.

Customer agrees to maintain monitoring service for the entirety of the selected term.

2. Payment Authorization

Customer agrees to:

  • Pay $36.00 per month for monitoring services

  • Maintain a valid payment method on file

  • Authorize automatic monthly billing

Failure to maintain payment may result in suspension or termination of services.

3. Early Termination

If Customer cancels monitoring service prior to completion of the selected term:

The remaining balance of the agreement will be due in full.

4. Activation Requirement

Monitoring services will not be activated until:

  • This Agreement is signed

  • A valid payment method is established

5. Equipment

  • Equipment may be purchased separately or as part of a package

  • Monitoring service is required for system functionality

  • Company is not responsible for issues caused by improper installation, misuse, or tampering

6. Service Limitations

Customer understands:

  • Alarm systems reduce risk but do not guarantee prevention of loss, damage, or injury

  • Monitoring depends on communication methods (internet, cellular, etc.) which may be interrupted

7. Limitation of Liability

Company is not an insurer.

Customer agrees that Company’s liability, if any, shall be limited to the lesser of:

  • $250, or

  • Six (6) months of monitoring fees

8. Entire Agreement

This Agreement represents the entire understanding between Customer and Company and supersedes all prior discussions or agreements.

Authorization

By signing below, Customer acknowledges and agrees to all terms of this Monitoring Agreement and confirms the selected monitoring term.
